• Completed Building Permit Application
• Completed Contractor Affidavit and copies of all licenses (or Homeowner Declaration if the work is to be done by the homeowner) 
• Fee payment in the form of Cash, Check, MasterCard or Visa. (Mail-in applications MUST be paid in Check form)
